About Sublime Care
Established in 2019, we began with a small but dedicated team, united by a shared vision: to be a guiding light for people with disabilities, helping them navigate the complexities of the NDIS with ease. Today, we employ 100+ employees across the Hunter and Western Sydney regions, who provide person-centred support services to over 100 participants in the following disciplines:

  • Community Care
  • Accommodation – SIL, SDA and ILO
  • Support Coordination, and
  • Allied Health.

 

 

About the Role

Reporting to the Allied Health Team Leader, you will use your therapy expertise and play a crucial role in supporting participants of all ages improve their independence and quality of life. Collaborate with others to ensure holistic and comprehensive care for their clients.

You will have outstanding prioritisation skills and the ability to work to arrange your schedule sufficiently. Your written and verbal communication skills will be advanced, and you’ll be confident in your computing/software skills.

 

 

Skills & Experience

  • Conduct Functional Capacity Assessments (FCAs)
  • Prescribe effective Assistive Technology
  • Assess and recommend home modifications
  • Implement evidence-based practices into ongoing therapy plans
  • Regularly review therapy outcomes and adjust interventions as needed
  • Prepare comprehensive reports, including NDIS plan review reports
  • Educate and engage with participants, stakeholders, family members, and frontline staff
  • Provide services through various modes: in-person, remote, and Telehealth.

 

 

Criteria

  • Bachelor’s degree in occupational therapy
  • Current AHPRA registration and First Aid Certificate
  • Current Driver Licence
  • Current NDIS Worker Screening Check and Working with Children Check
  • Evidence of COVID-19 Vaccination (minimum two doses)
  • Experience working with a range of clients aged 0 - 65 with varying support needs
  • Experience supporting NDIS participants with disability and/or mental health
  • Clear understanding of the NDIS Framework and Practice Standards
  • Strong verbal and written communication skills to engage with diverse participants and stakeholders
  • Expertise in conducting FCAs, recommending home modifications and writing complex reports.
