The Role 

As a Coordinator of Supports at Sublime Care, you are dedicated to making a positive impact on individuals with disabilities, chronic illnesses, and mental health challenges.

You advocate for and assist participants explore and cultivate their skills, abilities, and aspirations with sensitivity and confidentiality.

You excel in delivering exceptional customer service to participants, their support networks, and families.

You foster a respectful, and empathetic, approach, combined with strong attention to detail and accurate administrative skills.

  • Assess clients' needs and strengths and collaborate to create a plan of action aligned with their NDIS goals.
  • Monitor and evaluate progress, make adjustments as needed, and engage in plan re-assessments and reviews.
  • Coordinate with external service providers and community resources to offer holistic support, including social services and employment opportunities.
  • Advocate for clients’ rights, empower them in decision-making
  • Maintain detailed client records and progress reports, whilst effectively budgeting and billing time spent.

  • Experience in case management or support coordination.
  • Working knowledge of the NDIS, including how to maximise budgets.
  • Strong interpersonal skills for engaging with diverse individuals.
  • Self-motivation with drive to work independently and support to grow a developing department.
  • Excellent time management, administration, and prioritisation abilities.
  • Proficient computer skills and ability to learn/use multiple systems. .
  • Driver Licence, NDIS Worker Screening Check, Working with Children Check, COVID-19 Vaccination x two.
