  • Community Care
  • Accommodation – SIL, SDA and ILO
  • Support Coordination, and

Your Role

NDIS tells participants: “A Support Coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.”

 

We know how imperative this role is for participants. Without you, many people with disability and mental health might not flourish. It’s not a role for the faint hearted! Your role will include:

  • Engage with Participants to refine their goals and explain how to use their NDIS plans effectively.
  • Coach Participants in capacity building to increase their independence.
  • Communicate with a Participant’s stakeholders to help facilitate achievement of goals.
  • Engage and coordinate service providers on behalf of Participants.
  • Monitor the progress of Participants and their plan utilisation.   
  • Coach Participants how to access the NDIS portal and prepare for their next NDIS review.
  • Assess and manage risk in line with organisational Policies and Procedures.
  • Maintain accurate and up to date case notes.
  • Manage a varied caseload ranging from standard support, Mental Health, and Crisis Management support.
  • Participate in psychosocial coaching and other coaching as required.

 

 

Your Skills and Experience

  • Demonstrated experience in Support Coordination including Mental Health and Crisis Management.
  • Relevant tertiary qualification in disability, community services or case management.
  • Proficient understanding of the National Disability Standards, NDIS Practice Standards and NDIS operations, including support types and processes.
  • Demonstrated ability to provide professional and adaptive communication and customer service skills.
  • Highly developed time management skills to manage and meet demand of multiple tasks, competing priorities and deadlines.

 

 

To work with Sublime Care, you will also be required to provide evidence of the following:

  • Australian Working Rights
  • NDIS Worker Screening Check
  • Working with Children Check
  • Driver Licence
  • COVID-19 Vaccination (minimum two doses) 3.
